InterCommunity Health Care provides integrated health services to men and women recently released from prison in order to promote successful and healthy reintegration into their families and communities. Individuals receive comprehensive treatment for chronic medical conditions and peer support from a Community Health Worker who has a history of incarceration.

Individuals receive comprehensive treatment for chronic medical conditions and peer support from a Community Health Worker who has a history of incarceration.
To inquire about the program, contact the Bail Commissioner’s Office, Private Attorney or Public Defender’s Office, or Family Services of the Court where the client is in custody.
Those with charges or situations presenting an obvious risk to community safety will not be accepted.
No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income.
