InterCommunity Health Care provides several services for young adults (16-25) to help them:
Clients and their families can receive services regarding health, family advocacy, case management, education, peer support, and job support. Treatment teams help clients with housing, schooling, building community, and meeting their basic needs. This includes their social, emotional, spiritual, and cultural needs.
Clients can access care right in their community, at our clinics, or through our residential programs.
CT STAY STRONG provides community support, employment resources, and peer supports to transition young adults ages 16 to 25 who are at risk for significant behavioral health disorders. This program engages a team of wraparound staff that aids clients in building constructive relationships and support networks. Wraparound planning can address home and school needs, community connection, and social, emotional, or cultural needs. Clients receive services 2-3 times per month.
